This Coalition wants your voice to be included in efforts to strengthen connection and relationships between service providers and community members, and support paths to healing for community members impacted by Opioid use and addiction. This Coalition will use data, community-input and multi-disciplinary collaboration to set recommendations and direction for prevention, early intervention and treatment for those impacted by addiction.​

The Yolo Opioid Coalition was launched in October 2019 by CommuniCare Health Centers and Yolo County Health and Human Services Agency. The Coalition was created in response to the rising rates of Opioid use in the County and CommuniCare Health Centers expanding access to Medications for Addiction Treatment through funding from DHCS through the Hub and Spoke Model of Care, which included partnership with CORE Medical Clinics. Through this model, medication and treatment options expanded in Yolo County as well as education and awareness to providers, partners and community members. The Yolo Opioid Coalition was created collaboratively to bring various partners and community members together to share best practices, build relationships, address challenges together and to elevate and center the experiences of individuals, families and community members directly impacted by Opioid use.
